1. NEET Admit Card
2. NEET Score Card
3. All MBBS marksheets (First, Second and Final Professional) and Internship Completion Certificate
   OR
   University-issued MBBS Degree
4. Permanent registration from any State Medical Council. If permanent registration from Chhattisgarh Medical Council is unavailable, it must be submitted within three months.
5. Original domicile certificate (if required for eligibility)
6. Permanent caste certificate for SC/ST/OBC categories of Chhattisgarh State (if applicable)
7. For OBC Non-Creamy Layer candidates:
   • Income certificate issued by Tehsil Office for any one of the last three years
   • OR Form-16 issued according to standards fixed by the Government of Chhattisgarh
   • OR Self-declaration affidavit confirming non-creamy layer status as per General Administration Department guidelines
8. Disability certificate issued by Chhattisgarh State Medical Board for PwD candidates (if applicable)
9. EWS certificate issued by the competent authority after 01 April of the admission year and before 31 March of the admission year
10. Service certificate for in-service candidates (if applicable)
11. Migration certificate in case of university change, to be submitted within three months. If unavailable, affidavit may be submitted. Responsibility lies with the candidate.
12. Medical fitness certificate issued by District Medical Board/Medical College
13. Bond regarding compulsory service after completion of the course
14. Bond regarding course discontinuation penalty amount
15. One set photocopy of all documents
16. Aadhaar card/other valid photo ID proof such as driving licence/passport/voter ID
17. Three passport size colour or black & white photographs from the same negative
18. Bank guarantee for one-year fee for private medical colleges and applicable tuition fees
19. Documents related to NRI candidates
